Cost:

For medical use, there is no fee if records are to be sent directly to a doctor or other healthcare provider for the purpose of continuing care.

For copies for personal use there is a fee of $25.00.

For copies for other uses (attorneys, insurance companies, etc.) cost is as follows:

$25.00 for the first 10 pages and .10 each page thereafter.
